Building Use Application	Please use this form to request use of the building. (Submit this form at least 1 MONTH prior to the event.)
Member Connection Login Name and Password Request	Use this form to request a login name and password. This priviledge is reserved for members and regular attendars of Bethany CRC and is subject to approval of the leadership of Bethany CRC.
Prayer Request	Use this form to submit a prayer request or praise report to the prayer team. Can be submitted anonymously.
Sunday Announcement	Announcements that will be featured in Sunday Bulletin and in scrolling announcements. (Announcements are due at least 1 week prior to announcement date requested)
